
Dimapur, December 1 (MExN): The office of the Deputy Commissioner Dimapur on November 30 informed that a special enrolment drive for beneficiaries under Chief Minister Health Insurance Scheme (CMHIS)-General Category will kick off in Dimapur from December 4.
Accordingly, the Chairmen and GBs of all colonies under Dimapur Town were requested to informed public in their respective colonies regarding the schedule of the enrolment drive to be conducted from December 4- 14.
The authorities of colonies concerned were further urged to identify a suitable location for the enrolment drive dates.
Documents needed for CMHIS
1. Indigenous Inhabitant Certificate/Permanent Residential Certificate issued by competent authority of District Administration only after 2016 will be mandatorily required as KYC document during the enrolment drive.
2. For non-ration card holders, offline household verification form will be distributed by the ASHAS to the colony chairman and GBS before the enrolment drive. The form has to be filled and self-certified which will be verified by the colony chairman/GBs and countersigned by an administrative officer.
3 For ration card holders, direct collection of e-KYC in BIS portal and card generation will be done. A photocopy of Aadhar card and valid mobile number of the ration card holder and all family members are required as well.
