Kohima, September 30 (MExN): The temporary closure of PMJAY eCard generation on account of COVID-19 has been revoked in the larger interest of the public.
Informing this in a press release, State Health Agency (AB-PMJAY), Directorate of Health and Family Welfare, Government of Nagaland said that public can check their eligibility and apply for eCard generation at any of the authorised centres including Common Service Centre (CSC)-VLEs, and offices of Clairvoyance Technologies Pvt Ltd (CTPL). However, mass registration through BIS drives remain prohibited until further notice.
It also stated that COVID-19 safety precautions should be maintained at all times during eCard generation: Wearing of mask by beneficiary and PMAM/VLE is mandatory; beneficiary should sanitize hands before and after taking fingerprint for Aadhaar verification. PMAM/VLE should frequently sanitize the fingerprint device; physical distancing should be maintained and crowding should be avoided at the counter.
How to check eligibility and apply for PMJAY eCard:
• There are multiple channels to check your eligibility:
• Approach any empanelled government hospitals, and enquire with the Arogya Mitra at the PMJAY Helpdesk
• Common Service Centre (CSC) at your locality.
• Go to mera.pmjay.gov.in enter your mobile number and corresponding OTP, search with your name and/or given search parameters.
• Download PMJAY App from Google Play Store.
One may also approach any of the empanelled government hospitals, CSCs or Clairvoyance Technology Offices with the required documents. Every eligible beneficiary must provide a personal ID proof AND a family ID proof for issue of AB-PMJAY eCard.
Documents needed to produce for verification
Personal ID (any one of the below): Aadhaar Card, Voter ID, Pan Card and Any Government issued ID Card.
Valid Family ID (any one of the below): Ration Card and Letter from Panchayat/Village Council clearly listing out all the members in the household with photos pasted and signed across by Chairman Panchayat/Village Council (in the prescribed format available with Arogya Mitra/CSC)
For RSBY category of beneficiaries, family ID should be scanned together with RSBY Card.
